NorthBay Health informed the affected persons that an “unauthorized entity” gained access to the firm’s computer systems between January 11 and April 1 last year.
Thousands of Records Revealed
An internal probe reveled that the breach exposed thousands of records of individuals including names, birthdays, Social Security numbers, biometric information, passport numbers, financial account numbers, medical information, health insurance information, driver’s license numbers and state or other government-issued identification numbers.
Other highly sensitive data include usernames and passwords, credit or debit card numbers, expiration dates, security codes and personal identification numbers (PINs), the website said.
Following the revelations, the firm has urged the impacted customers to keep a close watch on their credit reports and account statements and offered a free one-year membership to Experian IdentityWorks, an identity theft protection service from Experian, a credit bureau.
