Notice of Privacy Incident March 26, 2025
Bellingham, Washington – In an abundance of caution, Northwest Radiologists and Mount Baker Imaging ("Northwest/MBI") are providing notification of a recent privacy event. Northwest/MBI experienced a network disruption on or about January 25, 2025, that impacted certain systems and immediately began investigating with the assistance of third-party computer forensic specialists to determine the nature and scope of the event. The investigation determined that certain data was accessed by an unauthorized party from January 20, 2025, through January 22, 2025. Northwest/MBI is currently reviewing and analyzing the types of information potentially impacted by this event; however, Northwest/MBI has confirmed that a limited amount of protected health information may have been impacted in connection with this event. Although the review is ongoing, the types of information impacted by this event might have included individuals' first and last names in combination with address information, telephone number, date of birth, email address, Social Security number, driver's license or state identification card number, treatment or diagnosis information, provider name, medical record number or patient identification number, health insurance information, and/or treatment cost information.
In response to this event, Northwest/MBI has implemented additional security measures intended to prevent similar incidents from happening in the future and will continue to monitor and enhance its security measures. Although Northwest/MBI has no evidence of actual misuse of information as a result of this event, Northwest/MBI notifying potentially impacted individuals out of an abundance of caution.

Additionally, it is always prudent to review health care statements for accuracy and report any services or charges that were not incurred to the provider or insurance carrier. As a best practice, individuals are encouraged to remain vigilant against incidents of identity theft by reviewing credit reports, account statements, and explanation of benefits forms for suspicious activity and to detect errors.
